| 目的：基于Meta分析方法系统地评价针刺四关穴对于缓解偏头痛患者疼痛的临床疗效。方法：知网、维普、万方、生物医学文献库、Cochrane图书馆、PubMed、Embase、WebofScience数据库中检索所有有关针刺四关穴治疗偏头痛的临床随机对照试验(RCTs),利用统计软件RevMan5.4提取并进行数据分析。结果：共纳入11篇相关文献，共包含897例患者，其中试验组451例，对照组446例。合并效应量显示：临床总有效率[RR=1.18,95%CI(1.08,1.29),P=0.0003],疼痛视觉模拟量表(VAS)评分[RR=-0.89,95%CI(-1.38,-0.39),P=0.0004],发作次数[RR=-0.79,95%CI(-1.11,-0.48),P<0.00001],持续时间[RR=-1.07,95%CI(-0.87,0.87),P=0.005],偏头痛程度[RR=-0.45,95%CI(-0.75,-0.15),P=0.004]及伴随症状评分[RR=-1.07,95%CI(-0.87,0.87),P=0.005],两组比较差异均有统计学意义(P<0.01)。结论：Meta分析显示，相对于单纯针刺疗法或药物疗法，针刺四关穴结合针刺或药物疗法可以显著提高偏头痛的临床疗效，在一定程度上降低偏头痛发作次数，缩短偏头痛发作持续时间，缓解疼痛程度，减轻伴随症状。 |

| Objective To systematically evaluate the effect of acupuncture at Si - guan point on relieving pain in migraine patients.Methods Randomized controlled clinical trials (RCTs) of acupuncture at Siguan point in the treatment of migraine were searched by computer in CNKI , VIP , WFD , CBM , Cochrane Library , PubMed , Embase and Web of Science. Statistical software RevMana 5. 4 was used to extract and analyze the data. Results A total of 11 articles involving 897 patients was included , with 451 patients in the observation group and 446 in the control group. The combined effect size shows : Total clinical response rate [ RR = 1 . 18 , 95% CI ( 1 . 08 , 1 . 29) , P = 0003] , Visual analogue Scale (VAS) score [ RR = - 0. 89 , 95% CI ( - 1 . 38 , - 0. 39) , P = 0004] , Number of attacks [ RR = - 0. 79 , 95% CI ( - 1 . 11 , - 0. 48) , P < 0. 00001] , duration [ RR = - 1 . 07 , 95% CI ( - 0. 87 , 0. 87) , P = 0. 005] , severity of migraine [ RR = - 0. 45 , 95% CI ( - 0. 75 , - 0. 15) , P = 0. 004] and accompanying symptom score [ RR = - 1 . 07 , 95% CI ( - 0. 87 , 0. 87) , P = 0. 005] , The difference between the two groups was statistically significant (P < 0. 01) . Con- clusion Meta - analysis showed that compared with acupuncture or drug therapy alone , acupuncture at Si - guan point combined with acupuncture or drug therapy can significantly improve the clinical efficacy of migraine , reduce the number of migraine attacks to a cer- tain extent , shorten the duration of migraine attacks , relieve the degree of pain to a certain extent , and alleviate the accompanying symptoms. |
